PA Consulting Group is proud of the benefits package offered to our employees.  Please take time to review all of the information contained in this enrollment guide.  We hope that you will find this material useful in making your benefit elections.  Benefits for newly hired employees are effective on the date of hire.

Eligibility Requirements

Employees:  Employees are eligible to participate if they are full-time employees of the Company and its domestic subsidiaries who are in active service and who are regularly scheduled to work at least 24 hours per week.  Employees of the parent company with established residency in the United States are also eligible to participate provided they are in active service and regularly scheduled to work a minimum of 24 hours per week.  Temporary employees and employees who are regularly scheduled to work less than 24 hours per week are not eligible to participate in the Plan.  Employees of a domestic subsidiary of the Company which has its own active health plan are not eligible to participate in this Plan. 

Family Members:  Eligible family members (dependents) include the eligible employee's 

• Lawful spouse;
• Unmarried dependent child under age 26 (regardless of student status); and
• Unmarried dependent child, older than age 26, who is mentally or physically disabled, who cannot hold a self-supporting job due to such disability and who depends on the eligible employee for primary support and care (but only if you provide the Plan administrator or Meritain Health, Inc., with proof of your child's disability -- at your expense -- no later than thirty (30) days after your child reaches age 26)
• Unmarried children of enrolled dependent children

Domestic Partners:  An eligible "domestic partner" means a person regardless of gender, named in the Domestic Partnership Certification Form that has been filed with and accepted by the Company, but only if that person:

• Is not married to anyone;
• Is at least 18 years of age;
• Is not related to you by blood closer than would bar marriage in the state where you and your domestic partner reside; 
• Is mentally competent to consent to contract;
• Has not had a different domestic partner in the past six months (except by reason of death); and
• Has resided in the same household with and shared the  common necessities of life with you for a period of at least six months prior to enrollment, and intends to remain in such status indefinitely
