Eligibility Requirements

Employees:  Employees are eligible to participate if they are full-time employees of the Company and its domestic subsidiaries who are in active service and who are regularly scheduled to work at least 24 hours per week.  Employees of the parent company with established residency in the United States are also eligible to participate provided they are in active service and regularly scheduled to work a minimum of 24 hours per week.  Temporary employees and employees who are regularly scheduled to work less than 24 hours per week are not eligible to participate in the Plan.  Employees of a domestic subsidiary of the Company which has its own active health plan are not eligible to participate in this Plan. 

Family Members:  Eligible family members (dependents) include the eligible employee's 

Domestic Partners:  An eligible "domestic partner" means a person regardless of gender, named in the Domestic Partnership Certification Form that has been filed with and accepted by the Company, but only if that person:
